- *QUALIFICATIONS:
- Education: Successfully graduated from a nationally accredited associate’s degree or baccalaureate degree program in nursing, as well successfully graduated from a nationally accredited Master degree program in nursing. Graduate from an academic program, within the nursing specialty field of study that is accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
- Certification: 1) Possess current certification as a Family Nurse Practitioner by the American Nurses Credentialing Center (ANCC) OR American Academy of Nurse Practitioners (AANP) OR the recognized national nursing certification organization for the specialty area of practice as appropriate (for example, National Certification Board of Pediatric Nurse Practitioners and Nurses, American Nurses Association, National Certification Corporation for the Obstetric, Gynecologic and Neonatal Specialties). 2) Possess and maintain a current American Heart Association BLS Healthcare Provider certification, National certification in Ambulatory Care Nursing and Advanced Cardiac Life Support (ACLS).
- Experience: Must have direct hands-on experience in the last 3 years with computer operations, basic word processing, data entry, and use/maintenance of automated electronic medical records is required.
- Knowledge of a wide range of medical diagnoses, disorders/conditions and disease processes across the lifespan.
- Knowledge of preventive health maintenance/schedules, ambulatory care procedures, clinical practice guidelines, clinical support staff protocols (CSSPs), and education/teaching methods related to wellness, disease, and/or disability management.
- Knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, risks/benefits, and complications, in addition to, accurate administration of the pharmacologic agent, including dosage calculations as required. . Knowledge of the administrative requirements for proper documentation of patient’s condition including disease progress, acknowledgement of teaching, and follow up care.
- Knowledge of the requirement for appropriate documentation of episode(s) of care, management of the treatment plan, and interventions, to include, patient, family and/or caregiver acknowledgement, understanding and agreement with the plan of care and treatment.
- Knowledge that the patient and family are in control as the major caregivers, users of the health system, and decision makers regarding compliance with the PCM team’s care regimen.
- Ability to recognize emergent, urgent and/or deteriorating signs and symptoms after triaging patients and responds according to approved nursing protocols.
- Knowledge of professional care theories, principles, practices, disease process and procedures.
- Licensure: Possess current unrestricted license to practice as a Family Nurse Practitioner, FNP in one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
- Must be a U.S citizen and have the ability to pass a background check/security clearance.
DUTIES: The duties include but are not limited to the following:
- Provide a full range of outcome-based nurse practitioner services in accordance with privileges granted by the MTF (e.g., supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated).
- Perform deployment related health assessment and annual mental health assessment services that comply with applicable Department of Defense and Air Force policy and guidance, to include required standards of care and clinical practice guidelines.
- Performs and/or refers to case management, utilization management, disease management, and care coordination functions, as appropriate.
- Serves as a part of the Primary Care Manager (PCM) team within the Air Force Medical Home (AFMH) model of health care delivery. Will provide assistance in the full spectrum management of professional and patient care activities within their scope of practice.
- Assesses and identifies individual patient care needs. Applies evidence-based practice (EBP) and processes in making appropriate clinical decisions for pediatric, adolescent, adult, obstetric, and geriatric patients.
- Synthesizes data from a variety of sources and make appropriate clinical decisions.
- Coordinates patient care through a continuum and facilitates the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
- Effectively communicates and collaborates both orally and in writing with a diverse group of individuals (peers, healthcare team, staff, patients, families and caregivers), for the purpose of informing the healthcare team and partnering with patients and families on the plan of care, treatment(s), education and/or outcomes.
- Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.
- Ensures compliance with professional standards of care and practice (i.e., American Nurses Association (ANA), American Academy of Ambulatory Care Nursing (AAACN), The Joint Commission (TJC), Centers for Disease Control and Prevention (CDC), Occupational Safety and Health Administration (OSHA), etc.). Adheres to Health Insurance Portability and Accountability Act (HIPAA) guidelines, the Department of Defense (DoD) Health Information Privacy Program and DoD/Defense Health Agency (DHA)/Air Force/Military Treatment Facility (MTF) policies, procedures, and guidelines. Provides care within ethical and legal boundaries.
- Accountable for making patient care assignments based on the scope of practice and skill level of all assigned personnel.
- Assesses, screens, and/or triages patients via telephone or in-person per approved MTF protocols and/or CSSPs. Requires expert, skillful, rapid assessment and triage for patient problem(s) and needs both in clinic and over the telephone.
- Effectively communicates when screening or triaging patients via telephone, secure messaging and/or face-to-face encounters IAW the ANA’s nursing process: assessment, nursing diagnosis, identification of expected outcomes/goals, implementation, and evaluation.
- Documents appropriate disposition/management of the patient to include optimal time and location to report to the emergency department (ED), urgent care clinic, follow-up with PCM or PCM team, or homecare and follow-up care as required.
- Executes medical orders, requests diagnostic tests and places appropriate referrals as directed by the provider. Obtains both internal and external diagnostic tests and consultation results, notifying the ordering provider of any abnormal results, and arranging follow-up care.
- Administers oral, topical, optic, ophthalmic, rectal, vaginal, IM, SQ and IV medications after validation of provider order. Must demonstrate proficiency in medication administration prior to giving medications at the MTF. Demonstrates knowledge of how pain contracts are initiated and their role in pain management processes within the MTF. Ability to recognize adverse signs and symptoms after medication administration and emergently respond per MTF’s protocol.
- Performs procedures within credentials and scope of practice. Procedures may include, but are not limited to, catheterizations, visual acuity tests, EKGs, irrigation of eyes and ears, splinting, phlebotomy, nebulizer treatments, PFTs, and skills in setting up, operating and monitoring specialized medical equipment for procedures.
- Assists in stabilizing patients in a clinical emergency, ensuring appropriate care/transfer to a higher level facility is accomplished as clinically indicated.
- Assists providers with patient examinations, therapeutic measures and procedures. Works closely with and refers patients to the Behavioral Health Optimization Program (BHOP) as appropriate.
- Ensures proper documentation and coding of notes, procedures, and treatments.
- Identifies and reports cases which have public health significance per MTF policy.
- Ensures all equipment is properly cleaned and stored at the end of each work shift, preventive maintenance and routine cleaning are performed as directed per manufacturer’s recommendations and medications/supplies are replenished.
- Encourages team building and enhances work knowledge by cross-training/augmenting other units as appropriate.
- Participates in the orientation and training of newly assigned personnel, as appropriate.
- Serves/participates in committees, functions and other meetings as directed. Provides relevant and timely information to these groups, and assists with decision-making and evidence-based process improvements. Participates in customer service initiatives, and medical readiness activities designed to enhance health services.
- Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies, universal infection control and prevention practices/policies, and process improvement initiatives.
- Promote preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling
STAFFING & SCHEDULING:
- Duty Hours: Work schedule shall be between the hours of 0730 to 1630 Monday through Friday. A one hour uncompensated meal break will be given during the shift as determined by the clinic.
- Place of Performance: The work to be performed will be at Eielson AFB, Alaska, 99702.
