Apex is looking for a full time companioned and professional Medical Case Managers position. Due to additional funding Apex is seeking Medical Case Managers position for our Danbury and Torrington locations. The Case Manager is responsible for ensuring that clients receive the most appropriate level and type of care needed as per the Ryan White Program standards of care and in accordance with agency policies.

Responsibilities:

· Provide People Living with HIV (PLWH) with a comprehensive assessment of basic physical, medical and psychosocial needs.

· Evaluate and document assigned clients for program eligibility according to program criteria

· Develop a client-centered care plan at least every 6 months

· Complete and follow up on referrals as needed by clients

· Support clients with scheduling clinical and non-clinical appointments and lab work

· Maintain a complete patient medical case management record that includes intake information, care plan and case management services notesbased on client’s acuity

· Enter required information in databases within the allowed timeframe

· Attend required training per program requirements

· Conduct home visits as necessary

· Create and maintain area resources and referral networks

· Provide culturally and linguistically appropriate information, education and referral

· Advocate for clients and act as a liaison between medical providers and community resources

Qualifications:

· Bachelor’s Degree preferred + 2 years of experience in HIV care and prevention or Associate’s Degree in Social Service + 4 years of experience in HIV care and prevention considered

· Valid, unrestricted driver’s license, reliable automobile and current automobile insurance required by CT state law

· Bi-lingual English/Spanish a plus

· Culturally sensitive and experienced with marginalized communities

· Work well with a diverse population in a non-judgmental and supportive manner

· Have strong interpersonal skills

· Attention to detail and task oriented

· Meet high standards of ethical and social responsibility

Pay: $44,000.00 - $48,000.00 per year
