We are seeking a Registered Nurse (PRN)

Duties to include:

• Perform initial evaluation and periodic re-evaluation of patient's condition and recommend revisions of the patient's plan of care, as needed.
• Conduct employee's certified medication training as needed.
• Develop RN assessments for patient's plan of care.
• Provide services requiring substantial and specialized nursing skills.
• Coordinate services and inform the Case Coordinator(s) of significant changes in patient's condition and needs.
• Initiate appropriate preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety.
• Counsel the patients and family in meeting nursing and related needs.
• Prepare clinical notes in accordance with professional and agency specific standards.
• Supervise the care provided by the direct service worker(s) to their assigned patients.
• Contribute to the orientation and ongoing education of personnel.
• Participate in performance improvement activities and mandatory case conferences.
• Performing an instructional visit teaching/instructing patient.
• Obtaining a medical history, particularly as it relates to the present condition, from the client and/or family member(s) and medical records.
• Evaluating the client, family member(s) and home situation to determine appropriateness for services with the agency.
• Evaluating the client's environment to determine what assistance will be available from family members in caring for the client
• Hours are flexible, you can make your own schedule.

Requirements:

Licensed nurse in the state in which they are performing duties.

• Possess knowledge of Medicaid guidelines and regulations essential to PCA Services.
• Demonstrate proficiency in clinical documentation.
• Possess good organizational and time management skills.
• Possess strong verbal, written communication and interpersonal skills.
• Display initiative and the ability to work independently.
• Possess good problem-solving abilities.
• Current driver's license with auto liability insurance.
• Computer, internet access and email capabilities from home or other off-site location.

Education

• Graduate from an approved School of Nursing.
• Three (3) years clinical experience as a Registered Nurse.
• Home Health Care Nursing Experience
• Behavioral Health Experience
• CPR/First Aid

Physical Requirements

• Work is performed in patient homes and in an office environment.
• Work requires continual mobility.
• Continually communicates in person and over telephone systems with patients, staff, and other health care professionals; this requires good hearing and speaking skills..
