R.N./Case Manager – WIC/FCM

The Sangamon County Department of Public Health is seeking applicants to perform duties primarily in the WIC and Family Case Management and Better Birth Outcomes Programs. The WIC/FCM/BBO nurse promotes positive health outcomes for participants through nutritional assessment, breastfeeding promotion/support, and program referrals. Duties include home visits and case management of social issues. This position includes the flexibility of working three days per week from home and two days per week in the office. Employee must provide their own internet service and will be provided a department phone and laptop.

The applicant must have the ability to communicate with diverse families, develop rapport and foster open communication. The applicant must have the ability to work 1 late day (7:00 p.m.) per week. On rare occasions, in the event of a public health emergency, this position may be required to work evening and/or weekend clinics as assigned. Excellent communication skills, dependability, a cheerful and professional personality, data input skills and ability to multi-task are all important and necessary skills for this position.

This position requires a current State of Illinois registered nursing license in good standing, a valid Class D Illinois Driver’s License, use of personal automobile and proof of auto insurance. Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment. Applicant must have the ability to work at least 1 late day (7:00 p.m.). On rare occasions, in the event of a public health emergency, this position may be required to work evening and/or weekend clinics as assigned.

Work Hours: Monday-Friday: 3 days from 8AM– 4:30PM, 1 day from 8AM-7PM, and 1 day from 8AM-1PM

Salary & Benefits: $54,400.82 yr/$27.8979 hr; 2 weeks paid vacation, 3 paid personal days,12 paid sick leave days and 12 paid holidays per year. Individual and Family health, dental and life insurance options, retirement, and mileage reimbursement.
