Our Platform Coordinator will be responsible for the management, retrieval, and processing of incoming referrals through various referral platforms. In this role, you will be the coordinator between Pinnacle Home Care marketers, and referral partners, for all new transactions in Allscripts, Navi, Epic NCH, and Powerchart among others. Platform Coordinators will provide independent, direct support to Care Coordinators as part of Pinnacle’s Central Intake team.

Location: This is a remote role, but this person MUST live in Florida and be willing and able to drive to the Oldsmar office for initial training and subsequent, and occasional, team meetings with notice. Flights to accommodate travel, for this role, is not an option. 4023 Tampa Road Oldsmar, FL 34655

Schedule is Thursday through Monday- without exception.
  • Work shifts- Thurs & Friday 11a-7:30p
  • Sat & Sun 9a-5:30p
  • Monday 11a-7:30p

Qualifications/Education:
  • Must have a GED or High School Diploma, associate or Bachelor’s Degree in a related field preferred.
  • Must possess excellent prioritization and time management skills
  • Must be able to communicate effectively and clearly

Responsibilities:
  • The Platform Coordinator will need to have organizational skills with the ability to effectively prioritize and complete tasks to support Care Coordinators with platform referrals
  • Coordinates to process patient referrals from Navi, Allscripts, Epic NCH and Powerchart and other referral platforms
  • Must be detail-oriented, possess the ability to multi-task, and be open to cross-functionally training in all departments
  • Displays a high level of patient focus that works directly with Care Coordinators, Case Managers, Physicians/ PCP(s), insurance representatives, outside vendors, and other medical personnel
  • Pulls referral off platforms to accept/decline, Reviews documents, confirms, and processes the referral through in Intake using Pinnacle’s EMR
  • Responds to Care Coordinator inquiries regarding referral status timely
  • Responds to all inquiries with a resolution to concerns and or discrepancies
  • Coordinates and communicates with the appropriate team regarding any changes or updates to the patient’s payer source in a timely manner
  • Meet productivity goals as assigned by the direct Supervisor
-Complies with HIPAA regulations on all accounts. -Follows Pinnacle Home Cares Attendance Policy
  • All other responsibilities assigned by leadership

Requirements:

  • Address customer concerns in a courteous and timely manner
  • Strong organizational and time management skills
  • Working knowledge of EMR database: Allscripts, Navi, Epic NCH, Powerchart
  • Proficient with Microsoft Word, Excel, and Outlook software
  • Possess good customer relation skills and a commitment to providing quality service
