Position Summary
Primary responsibility is to assist the Home Care Regional Manager in planning, directing, monitoring and supervising regional office staff. Provides direct supervision to assigned regional office staff. May provide care coordination services within an assigned geographic area as necessary.
Duties & Responsibilities
- Assists in hiring, training, evaluation, and discipline and may be responsible for direct supervision of assigned regional office staff.
- May provide care coordination services to families eligible for DSCC services within a geographic area as assigned
- Assists in reviewing/processing Care Coordinator activities in compliance with all rules, policies and procedures as directed.
- Assists RM in meeting the operational needs of the Regional Office as directed.
- Provides consultation to other members of the multi-disciplinary team, utilizing skills and knowledge acquired from academic training and professional experience.
Qualifications:
Minimum Qualifications
Bachelor’s degree
- Based on position requirements, additional education, training, and/or work experience in the area of specialization inherent to the position, may be required.
Additional education, training and/or work experience in the area of specialization:
At least one year of experience in either nursing, social work, speech pathology, or related field.
