Job Summary

The coordinator’s duties include new referrals, running eligibility and benefits, and sending morning reports.

Responsibilities / Essential Functions

  • Takes incoming referrals on a timely basis and runs benefits.
  • Answers incoming calls, receives all intake information and enters the appropriate information from the intake form and our MR system.
  • Communicates incoming referral information with marketing team.
  • Shares daily updates of Home Health and Hospice Census with staff.
  • Every morning, reports accurate active census numbers.
  • Handles all matters related to these referrals, except for issues that require professional or clinical staff's judgment.
  • Advises the Nursing Supervisor of all new referrals and coordinate with DON to ensure home health or hospice services appropriateness.
  • Acquires DME requests for all incoming hospice patients.
  • Coordinates with DME company to ensure the patient receives equipment in a timely manner.
  • Coordinates all admissions.
  • Cooperates with the Scheduling Manager regarding the investigation of client complaints/concerns.
  • Cooperates with Scheduling Manager regarding new processes developed to improve workflow and efficiency.
  • Assist the Scheduling Manager with any schedule-related issues or assistance with the scheduling of staff.
  • Files all incoming documents timely and efficiently, keeping all HIPAA information secured.
  • Preparation of orders to be sent out to physicians for their signatures.
  • Receives and follows up on faxes received from answering services each morning.
  • Enforces the Agency’s disciplinary process without prejudice or discrimination.
  • Participates in in-service programs.
  • Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the client caseload as quickly as possible relative to the event’s occurrence.
  • Follows instructions, is punctual, and provides reliable attendance.
  • Additional tasks may be delegated.

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
