Certified Medical Assistant **WORK FROM HOME** Full Time

Basic Job Duties:

• Support practices SDOH efforts through follow-up on completed, but unaddressed, SDOH surveys.
• Regularly visit primary care practices physical locations and collect completed SDOH surveys, electronically documenting results and/or initiate contact with patients to follow-up on positive screens (if needed).
• Establish positive supportive relationships with physicians, CHAP/GFHC staff, and patients/families.
• Initiate direct contact with patients to clarify SDOH needs and create referrals to GFHC CHAP program as necessary.
• Follow CHAP referrals to completion and ensuring results and feedback are embedded in the practice’s EHR and communicated to A&D clinical colleagues.
• Eventual embedment of CMA staff into primary care practices on a regular basis based on determined need and future duties as assigned to SDOH data collection and workflow.
• Regular review of health plan and external party high-cost, high utilizer, or targeted patient outreach reports to identify patients best fit for care management and clinical or SDOH follow-up.

Requirements:

• MUST LIVE IN GENESEE COUNTY (Michigan)
• 90% Work from home
• 10% Logistical activities such as; face to face oreintation, site tours, CPR training, etc.
• Positive, interpersonal skills.
• Must be a Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) or Community Health Worker (CHW)
• Valid driver’s license
• CPR preferred
