CARE COORDINATOR

We are seeking a Care Coordinator who will work closely with the company President in key aspects of our business, described below.
DUTIES & RESPONSIBILITIES
I. INTERVIEWING/MATCHING CARE PROVIDERS:
· Conduct most care provider interviews (unless unavailable)
· Check references and keep written record of such information; inactivate/re-activate existing care providers as appropriate.
· Make all arrangements to place care providers on assignments, including establishing their pay and developing detailed instructions for directions to the care recipient’s home.
II. CARE COORDINATION:
· Fill openings on all existing and new care recipient cases.
· Address all changes and issues regarding care coordination, which are communicated by the care recipients.
· Attend to the needs of all assignments in a thorough manner; maintain a good rapport and relationships with care recipients, their families and with the care providers.
· Assists the Community Relations Manager with sales efforts, preparation for marketing events, dispatching circulars, and other activities as deemed necessary by the Community Relations Manager.
III. BILLING/CP PAYOUT:
· Gather information on attendance (from DPNs, call-ins, call-outs) at existing clients, start-ups at new client families, and the ending of assignments; relay such information as billing notes on a weekly basis.
· Gather pay information to care providers on a weekly basis, and pass on to the Accounting Department.
· Review the final care provider payout report weekly and confirm with Corporate Office.
· Tally the care provider payout and billing notes, including weekly start and end dates of all assignments.
IV. DATA MAINTENANCE and AFTER-HOURS:
· Comply with all company policies & procedures and with state regulations.
· Prepare for handling no shows and/or new assignments by soliciting dependable and trusted care providers to be on standby, keeping updated care provider lists and current short lists.
· Maintain the Options Home Care System™ and QuickBooks data and submit weekly reports to the Corporate Office as directed by Management.
· Ensure that all changes and updates to care providers and care recipients addresses and/or telephone numbers are made on a weekly basis.
MINIMUM QUALIFICATIONS
· High school graduate
· 2+ years’ experience in a home care or service coordination role.
· Demonstrated capability maintaining strict confidentiality
· Ability to generate leads and monitor referrals
· Strong typing and computer skills
· Well organized, accurate, and attentive to detail
· Excellent communication, public relations and follow up skills
COMPENSATION
· $18 - $25/per hour
· Eligible for performance-based bonus
