Are you an experienced Senior Living Assistant Director of Nursing with a passion for delivering high-quality care to patients? Connected HC, a leading recruiting and staffing firm, is currently partnering with our client, a rapidly growing home health organization near Dodge City, KS to find a dynamic and motivated Assistant Director to join their team.
As the Assistant Director, you will oversee the day-to-day operations of the branch, ensuring compliance with all state and federal regulations, and managing a team of skilled healthcare professionals, including nurses, therapists, and aides. You will work collaboratively with other healthcare providers to develop and implement patient care plans, and develop strategies to improve patient outcomes and increase patient satisfaction. You will directly report to the Director of Nursing.

Key Responsibilities:
- Manage and oversee the day-to-day operations of the home health branch, including staffing, scheduling, patient care, and clinical outcomes
- Ensure compliance with all state and federal regulations, as well as company policies and procedures
- Collaborate with other healthcare providers, including physicians, therapists, and social workers, to develop and implement patient care plans
- Develop and implement strategies to improve patient outcomes, increase patient satisfaction, and reduce hospital readmissions
- Mentor, train, and supervise a team of skilled healthcare professionals, including nurses, therapists, and aides
- Conduct regular performance evaluations of staff, and provide coaching and feedback as needed
- Manage the financial aspects of the branch, including revenue growth, cost containment, and budgeting
- Participate in community outreach efforts to promote our client's organization and educate the community about the benefits of home health services
- Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient and caregiver education and patient, caregiver, and family counseling. Prepares clinical notes as appropriate.
- Assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations.
- Collaborates on development and implementation of policies and procedures which guide and support the provision of care. Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient care.
- Monitors and participates in the QAPI Program and assures appropriate corrective measures are performed.
- Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
Requirements:
- Valid RN license in the state of Kansas
- Minimum of 3 years of experience in Senior Living
- Strong leadership and interpersonal skills, with the ability to build and maintain effective working relationships with staff, patients, and families
- Excellent communication skills, both written and verbal, with the ability to communicate effectively with a diverse range of individuals and groups
- Knowledge of state and federal regulations governing home health services, as well as experience with Medicare, Medicaid, and private insurance billing and reimbursement
- Familiarity with electronic medical records (EMR) systems, and the ability to use them to manage patient care and documentation
